A good investment: longer-term cost savings of sensitive parenting in childhood

Introduction
Good quality parenting in early childhood is reliably associated with positive mental and physical health over the lifespan.
ObjectivesThe hypothesis that early parenting quality has significant long-term financial benefits has not been previously tested.
MethodsDesign: Longitudinal study with follow-up from 2012 to 2016; UK multicentre study cohort. Participants: 174 young people drawn from 2 samples, one at moderate risk of poor outcomes and one at high risk, assessed aged 4–6 years then followed up in early adolescence (mean age 12.1 years). Measures: The primary outcome was total costs: health, social care, extra school support, out-of-home placements and family-born expenditure, determined through semistructured economic interviews. Early parenting quality was independently assessed through direct observation of parent–child interaction.
ResultsCosts were lower for youths exposed to more sensitive parenting (most sensitive quartile mean £1,619, least sensitive quartile mean £21,763; p < .001). Costs were spread across personal family expenditure and education, health, social and justice services. The cost difference remained significant after controlling for several potential confounders. These included demographic variables (family poverty, parental education); exposure to child abuse; and child/young person variables including level of antisocial behaviour in both childhood and adolescence, IQ and attachment security.
ConclusionsThis study is the first showing that more sensitive early parental care predicts lower costs to society many years later, independent of poverty, child and youth antisocial behaviour levels and IQ. The findings provide novel evidence for the public health impact of early caregiving quality and likely financial benefits of improving it.

Introduction
Autism spectrum disorders (ASD) are associated with high services use, but European data on costs are scarce.
ObjectivesUtilisation and annual costs of 385 individuals with ASD (aged 4-67 years; 18.2% females; 37.4% IQ < 85) from German outpatient clinics were assessed.
MethodsClient Service Receipt Inventory
ResultsAverage annual costs per person were 3287 EUR, with psychiatric inpatient care (19.8%), pharmacotherapy (11.1%), and occupational therapy (11.1%) being the largest cost components. Females incurred higher costs than males (4864 EUR vs. 2936 EUR). In a regression model, female sex (Cost Ratio: 1.65), lower IQ (1.90), and Asperger syndrome (1.54) were associated with higher costs.
ConclusionsIn conclusion, ASD-related health costs are comparable to those of schizophrenia, thus underlining its public health relevance. Higher costs in females demand further research.

Background:
In schizophrenia treatment-compliance is a strong predictor of outcome, it depends on a longer lasting therapeutic alliance. SMS (Short Message Service) sent via mobile phones is an adequate tool to establish therapeutic contingency as was shown in a study on bulimia. This programme compliments outpatient treatment and is based on an exchange of SMS-messages between patient and therapist. On a weekly basis, patients supply information on subjective well-being, sleep, social contacts, and attitude towards medication. The patient's status is then rated as improved, deteriorated, or unchanged compared to the previous week and an adequate feedback message is sent. The study aimed at assessing feasibility and acceptance of this intervention in schizophrenia patients.
Methods:Thirty-six patients during inpatient-treatment for diagnoses of schizophrenia and schizoaffective disorder were screened and found eligible for participation. Those who agreed to participation were assessed with the SCID for DSM-IV and received instructions towards the use of the programme, which started after discharge and was designed to last for 4 months in addition to outpatient-treatment as usual.
Results:Eight patients agreed to participation (7 male, 1 female, mean age 29 years), six of whom dropped out during the first three weeks, and only one patient completed the study.
Conclusion:In the presented study, patients suffering from schizophrenia showed a low acceptance rate of additional SMS support complimenting their regular outpatient-treatment. As opposed to our hypothesis and contrary to experiences with bulimia patients integrating an SMS intervention into the treatment of schizophrenia does not seem feasible.
Socioeconomic position and depression in South African adults with long-term health conditions: a longitudinal study of causal pathways

Aims.
There is convincing evidence that lower socioeconomic position is associated with increased risk of mental disorders. However, the mechanisms involved are not well understood. This study aims to elucidate the causal pathways between socioeconomic position and depression symptoms in South African adults. Two possible causal theories are examined: social causation, which suggests that poor socioeconomic conditions cause mental ill health; and social drift, which suggests that those with poor mental health are more likely to drift into poor socioeconomic circumstances.
Methods.The study used longitudinal and cross-sectional observational data on 3904 adults, from a randomised trial carried out in 38 primary health care clinics between 2011 and 2012. Structural equation models and counterfactual mediation analyses were used to examine causal pathways in two directions. First, we examined social causation pathways, with language (a proxy for racial or ethnic category) being treated as an exposure, while education, unemployment, income and depression were treated as sequential mediators and outcomes. Second, social drift was explored with depression treated as a potential influence on health-related quality of life, job loss and, finally, income.
Results.The results suggest that the effects of language on depression at baseline, and on changes in depression during follow-up, were mediated through education and income but not through unemployment. Adverse effects of unemployment and job loss on depression appeared to be mostly mediated through income. The effect of depression on decreasing income appeared to be mediated by job loss.
Conclusions.These results suggest that both social causation and social selection processes operate concurrently. This raises the possibility that people could get trapped in a vicious cycle in which poor socioeconomic conditions lead to depression, which, in turn, can cause further damage to their economic prospects. This study also suggests that modifiable factors such as income, employment and treatable depression are suitable targets for intervention in the short to medium term, while in the longer term reducing inequalities in education will be necessary to address the deeply entrenched inequalities in South Africa.

Reactive processes associated with multiphase flows play a significant role in mass transport in unsaturated porous media. For example, the effect of reactions on the solid matrix can affect the formation and stability of fingering instabilities associated with the invasion of a buoyant non-wetting fluid. In this study, we focus on the formation and stability of capillary channels of a buoyant non-wetting fluid (developed because of capillary instabilities) and their impact on the transport and distribution of a reactant in the porous medium. We use a combination of pore-scale numerical calculations based on a multiphase reactive lattice Boltzmann model (LBM) and scaling laws to quantify (i) the effect of dissolution on the preservation of capillary instabilities, (ii) the penetration depth of reaction beyond the dissolution/melting front, and (iii) the temporal and spatial distribution of dissolution/melting under different conditions (concentration of reactant in the non-wetting fluid, injection rate). Our results show that, even for tortuous non-wetting fluid channels, simple scaling laws assuming an axisymmetrical annular flow can explain (i) the exponential decay of reactant along capillary channels, (ii) the dependence of the penetration depth of reactant on a local Péclet number (using the non-wetting fluid velocity in the channel) and more qualitatively (iii) the importance of the melting/reaction efficiency on the stability of non-wetting fluid channels. Our numerical method allows us to study the feedbacks between the immiscible multiphase fluid flow and a dynamically evolving porous matrix (dissolution or melting) which is an essential component of reactive transport in porous media.

Background. Morphometric studies on the corpus callosum (CC) in schizophrenia have yielded contradictory results. The aim of the present study was to investigate magnetic resonance imaging (MRI) abnormalities of the CC in first-episode patients with schizophrenic psychoses.
Method. We assessed volumetric MRI in 31 patients with diagnoses of schizophrenia, schizophreniform or schizoaffective disorder (DSM-IV) and a maximum exposure to neuroleptics of 2 weeks. As a control group, 12 healthy age- and sex-matched individuals were included in the study. A whole body scanner at 1.5 Tesla was used to obtain 3D T1- and T2-weighted MR datasets. The data were evaluated semi-automatically (intracranial volume, total brain volume) and manually (CC) with the software NMRwin.
Results. Patients had smaller CC and CC subdivisions than controls. Schizophrenic and unaffected women exhibited larger total CC and rostral subdivisions than men in both groups. Handedness did not exert an influence.
Conclusions. Our findings are in line with other in vivo morphometric studies on the CC in schizophrenia. The larger CC area in women may reflect general gender-related differences in CC size as described in healthy individuals.
Dikerogammarus villosus (Amphipoda : Gammaridae) : another invasive species newly established in the Moselle river and French hydrosystems

Based on field observations performed in 1999 and 2000, the exotic amphipod Dikerogammarus villosus is now well established throughout the navigable sector of the River Moselle in France. D. villosus co-occurs with Gammarus pulex, G. tigrinus and D. haemobaphes, another recent exotic species found in this tributary of the River Rhine. The maximal size of individuals of D. villosus collected (15.7 mm) is lower in the River Moselle than in other basins, where this species could reach up to 30 mm (Neseman et al. 1995). Nevertheless it represents the largest gammarids found in the French sector of the River Moselle. Other particular traits are its wide phenotypic plasticity, varying from a melanic to a striped form, and its predatory behavior. These particular traits may favour the establishment of this species on a recipient ecosystem. The occurrence of D. villosus was noted in the Alsace Canal and in the River Rhône, showing that its immigration across French hydrosystems is still in progress.

The average optical properties of an ultra-thin surface reaction layer (SRL) during growth by pulsed chemical beam epitaxy (PCBE) can be quantitatively accessed by p-polarized reflectance spectroscopy (PRS), as demonstrated on the example of heteroepitaxial GaP growth. Under PCBE growth conditions, the surface of the substrate is exposed to pulsed ballistic beams of tertiarybutyl phosphine [TBP, (C4H9)PH2] and triethylgallium [TEG, Ga(C2H5)3]. The pulsed precursor supply causes a period in composition and thickness modulated SRL, monitored as a fine structure that is superimposed on interference oscillations, resulting from back reflection at the substratelayer interface with increasing layer thickness. The amplitude of this fine structure undergoes a period amplitude modulation and exhibits turning points at which the response to the first precursor pulse changes sign. The turning points can be characterized by the expression R4(Φmax)=R4(Φmin), which describes the maximal and minimal values of the temporally modulated phase factor in the SRL, using a four layer stack description. The positions of these turning points are not affected by the thickness of the SRL, which allows the computation of the average complex dielectric function of the SRL independent of its thickness. In the next step, the average thickness of the SRL can be extracted from the amplitude of the observed fine structure.
Diamond Growth Rates and Quality: Dependence on Gas Phase Composition

Diamond growth rates and quality were studied as a function of source gas composition and correlated with position on the ternary C-H-O diagram. The chemical potentials of carbon and oxygen change dramatically on either side of the H2-CO tie line, leading to large differences in the equilibrium distribution of species. These differences are reflected in the species flux reaching the diamond surface, and hence in the quality and growth rate of the diamond. In situ microbalance measurements in a hot-filament reactor show that the reaction rate is independent of the CO concentration, but decreases with increasing O2. Quality, as measured by Raman spectroscopy, increases as the C/C+O ratio in the source gases is reduced to approach the critical value of 0.5. The stability of the filaments to decarburizing and oxidation are correlated with the carbon and oxygen chemical potentials and hence to the position on the C-H-O diagram. A preliminary ternary diagram for the C-H-F system is presented.

In this paper we report the growth and characterization of wide bandgap chalcopyrite structure semiconductor ZnGeP2 epitaxial layers by open tube MOCVD and ZnGeP2 single crystals by high-pressure vapor transport (HPVT). The electrical and optical properties of these crystals have been investigated by Hall measurements, absorption spectroscopy, photoconductivity and photoluminescence spectroscopy. The onset of the absorption edge for HPVT ZnGeP2 at 1.99 eV is in accord with the pseudodirect bandgap of this material. Both the meltgrown crystals and epitaxial layers show extended band tailing in the transparency range of ZnGeP2 depending on the growth conditions. Recently we have shown that epitaxial ZnGeP2 films can be grown with excellent surface morphology by MOCVD on (111) GaP substrates. These films should be suitable for phase matched second harmonic generation utilizing CO laser radiation in thick film heterostructures.
